ShootFirst - Custom Player Order Form
To get your very own hand-painted ShootFirst subbuteo player, all you need to do is fill in this form. We will then get back to you by replying to the email address you specified in the form to confirm the order/price. If you order during a busy period there may be a queue, but once painting has started it should take no more than a week before completion. Once completed we will send you photos of the player and if you’re happy we will then expect the payment via Paypal. Once payment has gone through the player will be sent to you via first class recorded mail. If you are unsure of any field click on the (?) to find out more. Player Details - The Basics
First of all, we'd like to know your players name and number...

Name (?)
Number (?)
Please type in your full name as this will feature on the side of the box (see picture to the right). The surname will feature on the back of your players shirt providing you tick the 'Name' decal box below.

Decals
Shirt Sponsor (?) Number (?) Name (?)
Each decal is an extra £1, if left unticked, the sponsor and number will be painted on, however, the name will be left out as that is a decal-only feature.

Base Colour (?) Disc Colour (?)
These colours will also determine the primary and secondary colours of your box (Yellow and Light Blue used in example box).

Team Name (?)
Type the name of the team, the season and whether it's home or away (e.g. MANCHESTER UNITED 2009/10 HOME)

This will feature on the side of the player box (see picture).

Box Message (?)
This will feature on the side of the player box (see picture).



Roll-over the image to see an example of what your box will look like. The yellow & light blue parts are determined by your base and disc colour you select in the form. If you would like an image on the side you can email it to us after you submit your form.

Player Details - Appearance
Now we need to know about your players appearence...

Sleeves (?) Ankle Tape (?)
Select whether you want your player to have long or short sleeves. The ankle tape will be painted on the lower part of the sock.

Skin Colour (?) Hair Colour (?)
Please select the skin colour and hair colour which you would like your player to have. ShootFirst paint a generic hairstyle on the player based on the shape of the 2k4 figures hairline. If you require a custom hair-style (e.g. an Afro, Mohawk or Long Hair) please tick the custom hair box in the extras part of the form..

Boot Colour (?) Boot Make
Puma, Nike and Adidas are the only available makes at the moment because they're easily recognised on the boots.
Remember - If you'd like a non-league side printing we need you to email us photos of the kit for reference.

Player Details - Extras
If you want to add more features to your player to give him more characteristics please fill out this part of the form.

Custom Hair (?)
We use special modelling putty to create custom hair. Custom hair will be an extra £1.50.

Wrist Bands (?) Left Right

Captain
Tick this box if you want your player to be captain. If your kit is dark the captain band will be white, if the kit is light the captain band will be dark.
